Martindale PAT Certificate Pad fields include:
• Type of Appliance
• Serial Number
• Date Inspected
• Inspected By
• Item – Part of the appliance to be tested
• Test – PAT test to be carried out, i.e. visual, insulation test, etc
• Condition – Condition of the part of the item under test
• Pass/Fail – Tick box for item status
